Athletic & extra-curricular Forms

Upon completion of electronic ATHLETIC & EXTRA-CURRICULAR forms, please do one of the following:

  • Upload and email to the faculty/staff member requesting the form. In the subject line of the email please indicate:  STUDENT’S LAST NAME, FIRST NAME.
  • Bring forms to the elementary/middle school front office or the high school front office.  

Clinic Forms

Upon completion of electronic CLINIC forms, please a) upload and email to nurse@smaschool.org requesting the form, or b) bring them to the elementary/middle school front office or the high school front office.  In the subject line of the email please indicate:  STUDENT’S LAST NAME FIRST NAME

5300 medication permit

Students complete if requiring medication to be dispensed during school hours. Include the physician's Action Plan with this form.

5305-A FOOD ALLERGY PLAN

Students complete if asthmatic, with allergies, or possibly requiring the use of an epi-pen or inhaler. Include the physician's Action Plan with this form.

5305-B FOOD ALLERGY GUIDELINES & PARENT WAIVER
